The last inspection report was not posted.

A caregiver did not share information with another teacher informing her one child was leaving. This lead the teacher mis-counting the children she was responsible for.

A teacher was asked how many children she had. She stated 8. It was verified by CCR there were 10 children in care.

A teacher was asked how many children were in care and she could not answer how many children she was responsible for.

Two 4 month old infants were watching TV.

There were 5 infants in the classroom. The specified age group was 8 months. Ratio was 1:5 is should have been 1:4.

Information was not shared with an incoming teacher with how many children were in care in the classroom that they were taking over.

Two staff caring for children have not received their eligibility letters. One teacher was in the infant room the other was in the pre-k classroom.

Two infants not on table food did not have written feeding instructions signed by the parent.

There is no documentation available showing a grounds checklist has been filled out.

A teacher did not demonstrate good judgment by allowing a child under 5 to hold the bottle for a 4 month old infant while they were being fed.

An infant age 4 months was being fed in a bouncer while the teacher held the bottle sitting down in a chair.

There is not a second exit on the playground.

There is no first aid kit.

There is no plan for safe route for children to exit the building while going to the playground.

CPSC form from HHSC has not been signed.
