Infants were left unsupervised when the caregiver left the group to open the door to let inspectors into the building. Noted: There were two other caregiver supervising their groups in the same general area.

A child ran out of the building due to a teacher leaving a door partially opened. The caregiver who following the child and assisted them in returning to the group let her hand go and walked away without ensuring she was safely back in the classroom, resulting in the child running outside the building a second time.
