Cleaning supplies in the bathroom used by children were observed to be in reach of children. This was corrected at inspection when the cleaning supplies were moved to a more secure location, out of reach of children.

A portion of the chainlink fence on the play ground was observed to have come unattached from the frame causing a possible entrapment hazard for children in care.

Upon reivew of files it was observed that a child with a diagnosed allergy had an allergy plan that was lacking the parent signature. Furthermore, the epi-pen brought for the child was not in the original container and was not labeled with the date it was brought to the center. There was also no allergy notice posted for the child in any of the rooms the child spends time in.

A caregiver was observed supervising a group of 14 children where the specified age group was 2 yrs old. The ages of children were as follows: 1- 22 mo old 6- 2 yr olds 7- 3 yr olds Please note this was observed as children were laying down to nap. None of the children in the room were asleep.
