It was observed during inspection that children were left outside alone multiple times and the caregiver did not have adequate supervision as they were unable to have visual or auditory awareness of each child. Additionally, the caregiver was not in close proximity to the children outside to be able to intervene if necessary.

An infant in care was not strapped into an equipment, rocker, that has safety straps. **This was corrected when the provider strapped the child in to the equipment.
The fire drills have not been documented since October 2023. The severe weather drill and lock down drills did not have the required drills practiced and documented at least four each in 2023. The testing and checking of smoke detectors, carbon monoxide and fire extinguishers was not documented.
The bathroom used by children was observed having items within reach of children that state keep out of reach of children as well as there were medicine and other items that should be kept out of reach of children.
During the inspection children were observed playing on play equipment that was on concrete and did not have required use zone all around the equipment of at least 6 feet.
There operational policies did not include information to parents regarding infant safe sleep.

The primary caregiver was missing 2 hours of the required 30 clock hours in the annual training year for 12/2021 - 12/2022. **This was corrected at inspection when the provider moved over 2 1/2 hours from the current year to the last completed training year.
The documentation of safety drills including checking of fire extinguisher, carbon monoxide, and smoke detectors was not available for review at inspection.
A child was observed being placed in a high chair and eating without being strapped in.
The sign in and out logs for the last three months were not able to be reviewed at inspection.

Upon file reviews, one child's file had no immunizations in file for revew and another child's file did not have an up to date immunization record in file.
The last children's product safety commission form available for review was signed on 1/5/2021. **This was corrected at inspection when it was signed by the provider at inspection.
The operational policies does not have a policy or information regarding the promotion of physical activity.
Infant feeding instructions were not available for review for an infant in care.
The first aid kit was not complete as it was missing the guide to first aid and emergency care manual, gauze pads, and anti-septic wipes/solutions were missing.
An adult in the home did not have substitute caregiver qualifications for review but was left in charge as the main caregiver for at least 15 minutes.
The last children's product safety commission form available for review was signed on 3/28/2017. One pet did not have pet vaccinations available for review. The primary caregiver did not have documented fire drills since August 2020 on file. **The children's product safety commission form was signed at inspection and this is in compliance. A fire drill under three minutes was conducted at inspection and documented and this this is in compliance.
