The outdoor play area had various hazards observed. The swing set has a barrier that is protruding out that is a trip hazard and has a sharp edge. The use zone of the slide has a two wooden boards protruding from the loose fill. One has a sharp edge and is a pinch hazard. The other board is a trip hazard. One bathroom has a spray bottle of sanitizer that is in reach of children.
