A caregiver was observed with children on a buggy ride around the street circle off the operation's property and was not carrying the required medical consent forms or emergency contact information for each child present on the ride. This was corrected during inspection by the director stating that buggy rides off property will no longer be conducted.
The loose-fill at the playground was below the required 9 inches for the large play structure.
Based on the information obtained during the investigation, it was determined that there was sufficient evidence for violation of this standard. The operation created an incident report for a parent with lines for signatures and dates but not ensuring that the signatures and dates were being completed. The operation's parent handbook states that court records, if any, must be on file for children enrolled, and one child did not have an existing court record on file.
Based on the information obtained during the investigation, it was determined that a child in care did not have a valid immunization exemption form on file or current immunizations.
A child stated that a caregiver pinched their chest when taking a toy away. A minor red mark was observed on the child's chest. The mark did not bruise and was gone by the next morning and determined to be a non-serious injury.
A training cup with drink in it was observed in the diaper changing area hand-washing sink.
The operation's annual fire inspection was past due. The last inspection was 3/10/22.
