Two infants ages 17 months old did not have any safety straps on when placed on their highchair. This was corrected during inspection.

Two infants feeding instructions were not updated during inspection. One was corrected during inspection when the parent of infant completed the form for licensing.

One out of four staff files checked was missing 11 hours of annual training from 10/28/2020 to 10/28/2021. Caregiver had 13 out of 24 hours.

Health statement was missing on file for two out of four records checked.

1 out of 3 staff records evaluated did not contain training in Preventing Shaken Baby Syndrome and Understanding Early Childhood Brain Development.

A baby walker with wheels attached was in the infant room. The provider removed the baby walker during the inspection.

A food allergy emergency plan did not contain a signature from the parent. Note: This was corrected during the inspection.

Paint is chipping on the wall in the older infant room and in the girls restroom.

3 out of 3 staff records evaluated did not contain training in the following topics: Responding to Food Allergies Administering Medication Handling, Storing and Disposing of Hazardous Materials

A yellow tunnel on the outdoor play structure is missing multiple screws and is detached posing a risk of injury to children.

Documentation of current transportation training for the director was not available for review.

There is no activity plan for the infants.
