There was no violation for the last inspection so condition nine did not apply but will apply next visit.

One of the staff files email to me did not have the required training hours. Caregiver only had four out of 24 hours of annual training from the period of 6/6/2021 to 6/6/2022. Director stated on her email to me that staff will complete her training 9/10 and 9/11.

I observed mixed age group during bedtime however out of the mixed age group two were older than six years old and were not separated from the other children.

Children were already having their bed time when I came into the operation however the Television was still on.

Based on the information obtained during investigation there is enough evidence to validate that a child was taken to Urgent care and management was informed but Licensing was not called.

746.1203(5) - Responsibilities of Caregivers - Children in Control

The operation did not provide the current fire inspection report.

During the inspection the children in the toddler and school-age classrooms were observed to be out of control. A toddler was observed attempting to climb on the diaper changing table and another toddler was observed pushing a chair around the classroom. In the school-age room several children were observed to be running around and hitting each other, one child was picking up another child and carrying them around the room. A third child used their knees to jump onto a child's back who was laying down on the floor which caused an injury that required first aid.

The first aid kits were reviewed for the 3 van used. The 1st kit did not have cotton balls, scissors, and a thermometer, the second kit did not have a first aid guide and a thermometer, and third kit did not have a thermometer.

In the infant room, a 14 month old was observed walking around the room playing with a ball and eating food. The food was removed from the infants hand and discarding after Licensing's presence was announced. In the school age room, several children were observed to be eating snacks and playing with toys or coloring, no handwashing was done in-between activities.

A caregiver with a group of children 14-21 months old was not aware of the ages of the children in their care. A caregiver with a group of children between 4-11 years old was not aware of the ages of the children in their care.

During the inspection, the caregivers were not aware of the number of children in their care. The caregiver initially stated there were 21 children in care then counted and stated there were 24 children. Later during the inspection, only 23 children were observed when asked where the 24th child was the caregiver stated they did not know because they were a substitute.

According to staff, children between the ages of 1-2 are placed in a corner for a period of 5 minutes.

A caregiver was asked for the number of children in their care. The caregiver stated there was 6 children in their care. When informed that there were 7 children in care, the caregiver began to ask where the child came from and stated they must have followed and child into the room. A caregiver was asked for how a count of the number of children in care. The caregiver stated that there were 6 children. When asked again the caregiver again stated 6. The caregiver was informed there were 7 children in care, they stated the child's parent was a caregiver in the next room and continued to say there were only 6 children.

During the walkthrough a 2yr old child was observed to be sitting in a high chair. According to the caregiver, the child was hitting several children and placed in the high chair for a few minutes.

A caregiver's diaper changing procedures was observed. The caregiver di not wash their hands after removing their gloves. A caregiver in the infant room was observed wiping a child's nose and did not wash their hands after. A child was observed to be changed and was not made to wash hands.

A three year old child was observed climbing into a booster table, the child was then observed climbing in and out of multiple booster seats in the table.

A caregiver with a group of children 15-17 months old was not aware of the ages of the children in their care. A caregiver with a group of children between 1-3 years old was not aware of the ages of the children in their care.

A caregiver on the playground with a group of children was observed sitting in a chair to prop open an exit door. In the toddler room, two nap mats were observed blocking an exit.

During the inspection the operation was asked for the record recently fired caregiver. The personnel record was not available for review.

There was no emergency contact information in the vehicles for the children who are transported.

The operation did not have an emergency preparedness plan available for review by Child Care Regulation.

Two out of four transportation vehicles did not have a fire extinguisher in the vehicle.

Three out of four transportation vehicles did not have a first aid kit in the vehicle.

The October 2021 fire drill was not documented. Note- This was corrected during the inspection when the fire drill was documented.

The personnel training records were not available for review by Child Care Regulation.

A play structure that includes a small slide did not have the required six feet of use zones maintained around the structure.

Two out of ten children's records evaluated did not have up to date immunization records on file.

A caregiver was allowed to have direct access to the children prior to receiving the results of an FBI background check. Note- This was corrected during the inspection when the caregiver was sent home until the results of the background check are received.

One caregiver was left with 31 children in the school age classroom during the follow up inspection. The child to caregiver ratio should have been 1:26. The operation was over in ratio by 5.

During the follow up inspection 2 caregivers were not aware of the number of childrern in their classrooms. One stated there were 14 children and we counted 15. In another classroom the caregiver stated there were 35 children and we counted 31. Note: I discussed the importance of knowing how many children are in each group so all children are accounted for.

Training certificates were not availale for review upon request during the follow up inspection.

An infant was observed in a crib with a pacifier with an attachment to it while in the crib. The caregiver immediately removed the pacifier during the inspection after I explained the risk of choking.

A caregiver stated the oldest child in the toddler room was 3 years old. The oldest child was actually 30 months old. Note: It was discussed with the caregivers how important it was to ensure they know the ages of each child in care.

Based upon the information obtained, there is sufficient evidence to support children are put in high chairs when they do not want to take a nap.

Based upon the information obtained, a caregiver walked out of the toddler classroom leaving children unsupervised for a short period of time. Note: Caregiver is no longer employed by the operation.

A caregiver in charge of the 2-3 year old class did not know how many children she was responsible for or the name of the child that was not signed in on the sign in and out log.There were 12 children in care but the caregiver stated there were 14 children in care. There were also 2 childre not signed in. Note: The caregiver signed 2 children in that were not signed in during the inspection and recounted the children to get the correct number of children in care.

Based upon the information obtained, there is sufficient evidence to support the operation put a child in a high chair to keep them from biting other children. Note: The child was dismissed from the operation.
