One bottle milk given to one infant did not have the infant name on it. This was corrected when the director wrote the infant name on the bottle.

The bottom dirt of the outside play area use zones were observed. Six inch or required mulch was not provided.

The caregiver in charge of a child who received minor scratches while in their care failed to notify the incoming caregivers of child's injuries and provide information of how the injuries happened.

Based on the information obtained during the investigation, children are placed in highchairs for discipline.

A caregiver responsible for 9 children (22m-3y) did not know the number of children present in their classroom. Another caregiver responsible for 8 children (2y) also did not know the number of children present in their classroom.

Two of three children records evaluated were missing the following from their admission forms as specified in subchapter 746.605: (3)Date of the child s admission to the child-care center; (6)Address of another responsible individual (friend or relative) who should be contacted in an emergency when the parent cannot be reached; 13) A statement of the child s special care needs. This includes, but is not limited to, allergies, existing illness, previous serious illness and injuries, hospitalizations during the past 12 months, and any medications prescribed for continuous, long-term use.

Observed a sand table filled with water and accessible to the children on the operation's playground. There were children between the ages of 2 and 5 years old playing on the playground. Note: This was corrected during the inspection by placing a top over the body of water, making the water inaccessible to children.

I observed a caregiver in the infant room rinse a child's bottle in the sink that is used for handwashing.

An infant bottles were not labeled with the child's first and last name. Note: This was corrected during the inspection.

There are two ant piles on the operation's playground.

The loose-fill surfacing materials on the operation's playground does not contain the proper depth.

Observed a water hose on the operation's playground that was not rolled up and secured which will cause a tripping hazard. Note: Correction was made during the inspection.
