An infant caregiver was observed not changing gloves in between diaper changes and not washing their hands after diapering. The caregiver used hand sanitizer only.

Two out of four bottles observed on the counter were not labeled with the child's name for identification.

Two infant children were observed not having their hands washed after being diapered.

A school-age child was observed in the staff lounge that is adjacent to the kitchen instead of a classroom. NOTE: The child was removed from the space and went home during the inspection.

One out of ten child files evaluated was missing updated vaccination records.

A 15 month old child was left without supervision on the playground.

Four infant feeding plans evaluated for children ages 5 mos, 5 mos, 10 mos, & 10 mos did not have a current plan on file.

A box of baking soda was accessible to children in the bathroom.

Two 3 year old children exited the playground and crossed 4 door thresholds resulting in exiting the front of the building into the parking lot without an adult.

Parent signatures were not obtained for the incident reports completed.

The operation completed an incident report 5 days after the incident occurred.

The last recorded fire drill was conducted on 7/21/22.

3 out of 10 staff files reviewed did not have a CPR certificate on file.

5 children were observed sitting in bucket seats to eat breakfast in the infant room and the seat belts were not secured around the children. NOTE: This was corrected when the caregiver secured the children in each seat using the available safety straps.

3 out of 10 staff did not have a First Aid certificate on file.

1 out of 10 child files evaluated was missing a current vaccination record. The child is 3 mons of age and is missing their DTaP, IPV and PCV vaccinations.

The director was made aware by a parent that there was an allegation a child was potentially placed at risk and did not notify licensing of the concern.

The operation did not report to licensing within 48 hours that a child who was in care obtained medical attention. An Administrative Penalty will be recommend as a result of this citation, per HRC 42.078.

Once child with a food allergy plan on file did not have the original prescription box on file for their prescribed medication.
