There were four children in the infant room, two children in the 4-5 room and 8 children napping in the toddler room. Only one staff member was present to supervise all classrooms.
One infant age child and three infant age children were asleep in high chairs.
Based on the information obtained during the course of the investigation staff has not demonstrated competency to perform assigned duties such as providing a consistent positive environment for children and comforting young children.
Based on the information obtained during the course of the investigation, Director has failed to provide leadership and demonstrate an understand of Minimum Standards. This has resulted in high-risk deficiencies including safe sleep and supervision.

Six out of twelve children napping were napping directly on the carpet. Mats were labeled with different children's names and solid color mats did not have a way to differentiate the floor from the napping side.

A 10 month old infant was observed sleeping in a high chair. Infant had a blanket covering their body except the head as observed during inspection. NOTE: This was corrected during inspection as child was moved to their designated crib by caregiver.
Four school age children were observed watching a video of Happy Tree Friends not related to planned activities on a tablet. A school age child was playing a video game not related to planned activities in an electronic device. NOTE: This was corrected during inspection as electronic devices were removed by director.
Written feeding instructions for two infants not ready for table food were not available for review.
Three caregiver records requested by Inspector were not available for review during inspection.
Infant daily report is not being completed for two infants at the operation. Caregiver confirmed report is not being provided to parents.
Caregiver left two infants unsupervised to be in the kitchen, room next door, completing other duties. NOTE: This was corrected during inspection as caregiver returned to the infant classroom and needed supervision was achieved.
Carbon monoxide detector was not present at the operation.
