Operational Policies were reviewed and the following information is missing: (a)(21) Procedures for parents to participate in the child-care center's operation and activities; (a)(22) Procedures for parents to review a copy of the child-care center's most recent Licensing inspection report and how the parent may access the minimum standards online; (a)(23) Instructions on how a parent may contact the local Licensing office, access the Texas Abuse and Neglect Hotline, and access the HHSC website; and (b)(1) You must inform parents that they may visit the child-care center at any time during your hours of operation to observe their child, the program activities, the building, the premises, and the equipment without having to secure prior approval.

Five staff files were reviewed. One file is missing annual training certificates and notarized affidavit. A second file is missing translation of foreign diploma.

Most recent health inspection was not available for review at the time of inspection. Inspection report provided by completed on 5/22/2023. Note: A copy of the most recent health inspection report (11/8/2023) was provider by operation via text message after the conclusion of the inspection.

Last fire inspection was conducted on 01/21/2022. Note: Provider contacted the City of McAllen and requested an inspection.

There was insufficient light in the toddler room during naptime. Note: This was corrected at inspection when provider turned on the light to the adjacent room allowing additional light to enter the room.

Two children in the toddler room were observied in high chairs while the rest of the group participated in a craft. One child in the prek room was observed a high chair while rest of the group participated in a craft.

One out of five staff trainings records reviewed was not available for review at the time of inspection.

Two caregivers were providing care for a total of 5 children ages 2mo, 3mo, 8mo, 9mo and 13mo. One of the caregivers stepped out of the room and remained in the kitchen during lunch preparation leaving one caregiver in charge of the 5 children.

There is mopping bucket containing water in the prek age room and accessible to children. Note: This was corrected at inspection when provider removed bucket.

One out of five staff records reviewed has an expired first aid/cpr training.

A child was placed to nap on a play-yard. The play-yard had an additional mat and foam layer which was added on top of the original play-yard mat and covered with a sheet. Note: This was corrected at inspection when provider removed additional mat and foam layer and placed a clean sheet on the original play-yard mat.

One staff member is missing training on Safe Sleep/Shaken Baby/Early Brain Development.

One staff file is missing copy high school diploma or equivalent.

Two children's files are missing immunization records and three are missing health statements.

United States Consumer Product Safety recall list was last certified on 06/19/2020.

During nap; a child's mat was placed on the hall leading to the playground blocking the emergency exit. Note: This was corrected at inspection when mat was relocated.

Four staff training records were reviewed. One file is missing 12 clock hours of training including trainings in the following topics: -Abuse/Neglect -Hazardous Materials -Food Allergies -Administering Medications -Emergency Preparedness -Building and Physical Premises Safety -Communicable Diseases Two files are missing training on -Safe Sleep/Shaken Baby/Early Brain Development.

Folder taken during transportation routes is missing the following: -List of Children Being Transported -Emergency medical transport and treatment authorization forms -Written Transportation Emergency Plan

Six children's files were reviewed. One file was not available for review at the time of inspection. Two files are missing immunization records. Three files are missing health statements.

Per provider a fire inspection was recently conducted, however, inspection report available for review was conducted on 11/6/2020.

Four staff files were reviewed. A file is missing high school diploma. A second files requires a high school diploma be translated into English. Three files are missing copy of ID/DL. One file is missing hired date, affidavit and statements regarding personnel/center policies and orientation.

Written activity plan for the infant classroom was not posted. NOTE: This was corrected at inspection as activity plan was posted by director.

Caregiver was showing cartoons/lullaby on cell phone to 4 infants in care as part of an activity. NOTE: This was corrected at inspection as caregiver put cell phone away and a different activity was initiated.

Written feeding instructions were not available for review for one infant in care not ready for table food.

A baby walker was observed in use by an infant during inspection. NOTE: Baby walker was removed from licensed child care area.

Written activity plans were not available for review for 2 groups of children: Infants, Pre-K and School-Age.

Two bottles were observed in the infant classroom without label with children's name. NOTE: This was corrected as name and last name initial was added to each bottle.
