Six staff training records were reviewed. Some of training certificates for five of the staff members were not available for review at the time of inspection.

Operational Policies were review and the following items are missing: -Physical and Outdoor Activity Policy -Inspect Repellent and Sunscreen Policy -Breastfeeding Policy -Vaccine Preventable Diseases Policy for Employees

One out of five children's records reviewed are missing health/physician statements.

Four out of five children's records reviewed are missing health/physician statements. Files are also missing a statement confirming parents received a copy of the operational policies.

Five personnel files were reviews. Two files are missing employment begin date, one file is missing high school diploma or equivalent, one file is missing notarized affidavit, four files are missing center policy statement, three files are missing personnel policy statement, three are missing copy of ID and four are missing orientation statement.

Two out of five staff training records reviewed are missing pediatric first aid/cpr training.

Operation has two playgrounds with a swing set and play structures with a height over 20 inches; surfacing surrounding the structures do not meet the require depth of 6 inches.

Five staff training records were reviewed; two are missing 19-24 hours of annual training including trainings on the following topics: Management Abuse and Neglect Safe sleep/Shaken Baby/Early Brain Development Hazardous Materials Food Allergies Administering Medications Emergency Preparedness Physical Premises and Building Safety Communicable Diseases

Initial background check was not ran for a caregiver upon hire. Caregiver was present during inspection. NOTE: This was corrected as caregiver's background check was ran during inspection. Caregiver left the operation during inspection until eligibility is determined.
