First Aid / CPR Training for two caregiver and Annual Training Certificates for six staff member were not available for review at the time of inspection.

Infant age children were observed in high chairs and bucket seats for 25 consecutive minutes. Note: During inspection, caregiver removed the children from the devices and moved them onto the indoor playground area.

Two out of ten children's files reviewed are missing emergency medical authorization, health statement and health provider information.

Operational policies were reviewed and the following items are missing: (17) Pets (18) Physical and Outdoor Activities (26) Abuse and Neglect (30) Inclusive Care

Children Products Certification Form was not available for review at the time of inspection.

Seven out of ten staff files reviewed are missing trainings, including trainings on the following topics: -Abuse/Neglect -Safe Sleep / Shaken Baby -Hazardous Materials -Food Allergies -Administering Medications -Emergency Preparedness -Building Safety -Communicable Diseases -Transportation -First Aid / CPR

Consecutive diaper changes were conducted without sanitizing the diaper-changing surface after each child.

A staff member was present at operation and providing care to children without prior background check eligibility notice. Note: This was corrected at inspection when staffed was sent home pending background check eligibility notice.

Three out of ten staff files requested for review were not available at the time of inspection. A fourth file is missing copy of high school diploma and fifth one is missing high school diploma and personnel / operational policies and orientation statements.

Statements of health and vaccination records for 3 pets at the operation were not available for review at the time of inspection.

Toddler room has a cabinet with a broken door, cleaning products are stored in the cabinet and are accessible to the children.

Based on the information obtained during the course of the investigation, a caregiver was left in charge of supervising 6 children ages 13-16mo. Specified age group was 12-17mo, classroom was over ratio by one. child.

Last gas leak inspection was conducted 03/2021.

Based on the preponderance of evidence, a caregiver failed to demonstrate self-control / good judgment and became impatient towards a child that was attempting to leave classroom.

Operation did not report to CCR an incident that involved inappropriate forms of discipline / guidance.

A caregiver grabbed, pushed, pulled and attempted to physically restrain a child that was attempting to leave classroom.

Two out of ten children's records review are missing a health statement.

Two staff members were caring for 11 children ages 8mo-18mo. Specified age group of 0-11mo. Maximum group size was over by one child.

Required Postings were reviewed and the following are missing: Most Recent CCR Inspection / Investigation Form Keeping Children Safe Poster List of Employees Note: Keeping Children Safe Poster and List of Employee were posted.

Operation has swings and a play structure that has slide. There is 2-4 inches of pea gravel surrounding the structures/play-structures. Playground a rotating active play equipment that is within 2 feet from a metal pole.

Ground checks are conducted daily by operation but operation has not documented monthly checks utilizing a general maintenance checklist or safety checklist.

Operation has four bucket swings; two of them do not have working safety straps and the other two inner inning is peeling. Swings are 14-16 inches from the ground.

Operation does not have a written transportation emergency plan.

Cots in the toddler were not labeled with the children's names and/or assigned number. Two out of the eight cribs being used in the infant room were labeled with the children's names.

Three medications were administered to a infant a child. Label instructions indicate medications are for children over the age of 2yo, 5yo and only with doctor's prescription. Operation did not have a written amended in writing by the child's health-care professional.

Two out of six staff training records reviewed are missing pediatric first aid/cpr. Note: one employee will no longer be working as a caregiver or be left in charge of a group of children.

Three out of the thirteen children's records reviewed and missing immunization records.
