Observed teacher 4 teachers doing diapering procedures and did not wash hands in between in child's diaper change. Teachers observed to be wearing gloves and would remove gloves prepare the area and place new gloves on rather than washing hands after area was prepared for new child in diaper change. Observed a caregiver wipping childs nose to remove buggers, teacher threw wipes that was used to clean nose away and grabbed a new wipe to clean another child's nose. Observed Class coming inside from outdoor time and teachers did not wash the childrens hands once they came back inside.

Based on the preponderance of evidence, a caregiver failed to demonstrate self-control / good judgment and became impatient towards a child that was attempting to leave classroom.

Operation has swings and a play structure that has slide. There is 2-4 inches of pea gravel surrounding the structures/play-structures. Playground a rotating active play equipment that is within 2 feet from a metal pole.
Ground checks are conducted daily by operation but operation has not documented monthly checks utilizing a general maintenance checklist or safety checklist.
Operation has four bucket swings; two of them do not have working safety straps and the other two inner inning is peeling. Swings are 14-16 inches from the ground.
Operation does not have a written transportation emergency plan.
Cots in the toddler were not labeled with the children's names and/or assigned number. Two out of the eight cribs being used in the infant room were labeled with the children's names.
Three medications were administered to a infant a child. Label instructions indicate medications are for children over the age of 2yo, 5yo and only with doctor's prescription. Operation did not have a written amended in writing by the child's health-care professional.
Two out of six staff training records reviewed are missing pediatric first aid/cpr. Note: one employee will no longer be working as a caregiver or be left in charge of a group of children.
