Rugs and cabinet with tape residue were removed from the school-age room. No toilet brushes / plungers were observed within the children's restroom.Six electrical outlets were observed uncovered: two in the multi-purpose room, three in the toddler room and one in the prek room. This was corrected at inspection with provider covered the outlets. The wall in the multi-purpose room and chipping paint within the rooms have not been fixed. There was a torn garbage bag within the playground and an uncovered sandbox.
Rugs in the school-age room have multiple stains and a cabinet has tape residue. A toilet brush and plunger were observed within the children's restroom. There is chipping paint on the walls of the front / multipurpose room and prek room. Six eletrical outlets were uncovered.
Children were not observed participating in the planned activities listed under the lesson plans posted.
A caregiver was observed changing a child's diaper in the pre-k room and placing the soiled diaper on the floor.
A careigver caring for a group of children did not have training in Pediatric first aid with rescue breathing.
A caregiver was caring for a group of children of varying ages including an infant. There was an 18 month age difference between the youngest and oldest child in the group. The youngest child was 13 months old and the oldest child was 3 years of age.
A group of children in the afterschool room were observed playing on tablets. The games the children were playing were not related to the planned activities.

There was no documentation available for review as required for a unitary surfacing meterial in the outdoor playground.
The following items were missing from the f first aid kit in one of the transportation vehicles: 1. Thermometer; and 2. antiseptic solution or wipes
There was no carbon monoxide detector at the center.
A caregiver caring for a group of children was missing Eight hours of pre-service training.
Trash such as a banana peel and plastic bag were observed in the outdoor play ground accessible to children. Cleaning supplies and plastic bags were observed under a children's sink. There was no latch on the doors preventing the cabinet from opening. Note: This was corrected during the inspection as the trash were removed from the outdoor playground and the cleaning supplies found under the sink were stored in an area out of children's reach.
The director was missing 3 clock hours of annual training in management techniques, leadership, or staff supervision.
The sign in and out log was not maintained as only 9 children were signed in but a total of 12 children were counted present. Note: This was CORRECTED during the inspection as the sign in log was updated with the missing information.

A caregiver with less than 90 days of employment was caring for a group of children without training in Pediatric first aid.
Nap mats observed to not be labeled with children names or any alternative labeling method such as a number list for each child or stickers on tags.
Naptime observed, children were sleeping on mats close to one another where not enough walkway or space between children. A caregiver observed to have the need to step over and on mats to reach to a child on opisite end of room.
Activity plan for toddler room was not avaiable for review.
