The Children's Products Certification form that is used to certify that the center has reviewed each of the recall notices issued by the CPSC was not current. Note: This was CORRECTED during the inspection as the CPSC website was reviewed and the form was completed.

The operation policies reviewed were missing the criteria the center will use to determine when extreme weather conditions pose a significant health risk that prohibits or limits outdoor play.

A caregiver caring for a group of children was still missing training in Pediatric first aid with rescue breathing and choking. Re-cited.

A caregiver's pediatric CPR training completed did not adhere to the guidelines for CPR for a layperson established by the American Heart Association. Re-cited.

There was no documentation available to verify that the center conducts monthly tests on smoke detectors and carbon monoxide detector or monthly checks on fire extinguishers.

The center did not have a carbon monoxide detector.

A container of disinfecting wipes, a can of lysol disinfectant spray, and a bottle of hand sanitizer were observed on top of a rolling cart in a toddler room. This posed a safety hazard to children as the cleaning supplies were within children's reach. Note: This was CORRECTED during the inspection as the cleaning supplies were stored out of children's reach.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.

A caregiver was missing annual training for training year 10/06/2020- 10/05/2021 on the following topics: (1)Recognizing and preventing shaken baby syndrome and abusive head trauma; (2)Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS); and (3)Understanding early childhood brain development. -Prevention, recognition, and reporting of child maltreatment; and (1) Emergency preparedness; (2) Preventing and controlling the spread of communicable diseases, including immunizations; (3) Administering medication, if applicable, including compliance with 746.3803 of this chapter (relating to What authorization must I obtain before administering a medication to a child in my care?); (4) Preventing and responding to emergencies due to food or an allergic reaction; (5) Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and (6) Handling, storing, and disposing of hazardous materials including compliance with 746.3425 of this chapter (relating to Must caregivers wear gloves when handling blood or bodily fluids containing blood?).

2 of 6 children's records reviewed were missing the child's emergency contact's address. 1 of 6 children's records reviewed did not have an emergency contact listed. Note: This was CORRECTED during the inspection as the required information was documented.

There was no Food Allergy Emergency Plan available for review for a child with a diagnosed food allergy.

A caregiver's education documentation received outside of the United States was not translated into English.

A caregiver completed standard CPR training online.

The operation did not have an annual health inspection completed. The last health inspection was completed on 2/07/2020.

Two electrical outlets did not have safety covers in a toddler room. This pose a safety hazard as the electrical outlets were within children's reach. Note: This was CORRECTED during the inspection as the safety covers were placed on the electrical outlets.

Fire drills were not practiced for the months of January and March of 2021.

Ratios were not being met in one of the infant rooms when a caregiver was left caring for 5 infants while the other caregiver was observed in the hall way. The specified age of the children in the group was 0-11 months. According to the ratio chart one caregiver may care for a maximum of four infants.

A 4 month old infant was observed to have a pillow placed under their head while sleeping in the crib. The pillow was removed from the infant's crib.

The diaper changing mat in one infant room and in a toddler room were observed torn.

The operation did not have a current annual fire inspection. The last fire inspection was completed on 2/05/2020.

The operation was over the infant capacity allowed on the permit. There were a total of 13 infants in care and the operation infant capacity on the permit is 12.
