There was a decomposed pumpkin on top of a fridge and a expired gallon of milk inside the fridge. A child's soiled underwear, shorts, and shoes were observed on top of the hand washing sink in a children's restroom. Note: This was CORRECTED during the inspection as the soiled clothing was removed from the children's restroom and placed in a sealed plastic bag.

Caregivers did not ensure continuity of care for children by sharing with incoming caregivers information about coverage during the caregiver's breaks. As a result 3 infants were left unsupervised in the infant room.
Three infants were left unsupervised in the infant room for an undetermined amount of time when the caregiver caring for the infants was observed in the afterschool room with another caregiver.
The director is not meeting director responsibilities as caregivers are leaving children unsupervised and not complying with minimum standards.

The lighting in the infant room and toddler room during naptime was not adequate to allow visual supervision of all children in the group at all times. Note: This was CORRECTED during inspection as the window curtains were moved to allow enough lighting into the rooms.

Ratios were not being met when there was more than an 18 month difference when older children and infants were combined. The oldest child was 10 years old and the youngest child was 16 months old.

In addition 5 toddlers were left unsupervised in the toddler room when the caregiver in charge of the group was also observed in the hallway outside of the room.
