Last fire drill and fire ext / CO detector checks were conducted / documented on 12/05/2024.

Operational policies reviewed, Promotion of indoor and outdoor physical facility section was missing in handbook.

One file reveiwed with missing immunization records in file.

Based on the preponderance of evidence it is determined that the Director in not present in the operation a minimum if 75% or 30 hours a week.

A review of the Employees Who Left Employment in 2023 data from January 18, 2024, revealed the center had not reported the number of employees who left employment in 2023 by January 15, 2023, as required.

A caregiver did not have current training in transportation safety. Note: This was corrected during the inspection as the caregiver took the training online during the inspection.

Training records for the director and a caregiver were not available for review,

The first aid kit in a transportation vehicle was incomplete. The following items were missing: (1)Tweezers; and (2)Waterproof, disposable gloves. Note: This was CORRECTED during the inspection as the missing items were obtained and placed in the first aid kit.

Three of Four children's files reviewed were missing the child's immunization record. One of Four children's immunization reocrd reviewed was not current.

Notification was not received when there was a change in the outdoor space. A storage shed was built in the children's outdoor playground.

3 of 6 children's records reviewed were missing the name and telephone number of the school that the school-age children attend. Note: This was CORRECTED during the inspection as the school information was documented on the children's admission forms.

The second director was missing annual training on Understanding early childhood brain development.

Two caregivers were missing annual training on the following topics: Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS); and Understanding early childhood brain development.

The lighting in the toddler room was not adequate to allow visual supervision of all children in the group during naptime. Note: This was CORRECTED during the inspection as the light in the diaper changing area was turned on.

A child was observed napping in the toddler room unsupervised.

2 of 4 childrens records reviewed were missing the child's current immunization information. One of the records was also missing the statement of the child's health from a health-care professional.

An employee that did not meet caregiver qualifications was observed caring for a group of children in the toddler room.

The Parent s names and telephone numbers and emergency telephone numbers were not available for two children being transported. Note: This was CORRECTED during the inspection as the required information was obtained and placed in the transportation binder.

The Emergency medical transport and treatment authorization forms were not available for two children being transported. Note: This was CORRECTED during the inspection as the required forms were obtained and placed in the transportation binder.
