An infant who cannot roll over on their own was seen sleeping on their stomach. This was corrected at inspection when the infant was awakened from the nap.

Two caregivers did not know the number of children they were in charge of. This was corrected when the caregivers explained a child had came in late and were able to identify which child.

There are two doors that have holes in them and are acessible to children.

Five of ten staff were observed to have online CPR training. The staff will complete an in person CPR training course to come into compliance.

It was observed that cabinets containing cleaning chemicals were unlocked and accessible to children in each classroom. This was brought into compliance during the inspection by utilizing the cabinet locks installed.

it was observed that four infants ages 6-9 months were left unsupervised in a classsroom with one 7 year old child when the caregiver left the classroom to use the restroom. It was also observed that one classroom of 12 two year olds was left alone unsupervised during nap when a caregiver answered the door and was walking classroom to classroom with the licensing inspector.

One child of a caregive was observed napping in a PreK classroom. There was no enrollment information on file for the child.

Two of six children's files reviewed did not have physician signed health statements.

While reviewing eight childrens files, two files did not have current immunization records.

During the inspection it was found that the operation is not documenting playground maintence checks.
