On 5/22/2023, there were two people present at the operation pending background checks and in care of children. One person was in the infant classroom and the other was in the 2 & 3 year old classroom.

Based on the information gathered and interviews conducted at the time of the incident the caregiver was wiping down shelves, cleaning and sweeping not directly involved with teaching, care and supervision of the children.

The Consumer Product Safety was last certified on 1.1.21. NOTE: This was corrected when the website was reviewed a CPS form was posted.

The PreK 4 classroom children were observed out of control as children were running, hitting/throwing pillows at friends, not keeping hands to themselves, child flipping on the carpet, stuffing the sink with paper towels as the caregiver was getting ready to pass out snack. In addition to the caregiver passing out skittles for showing good behavior, and cleaning up the classroom.

The Pre school and toddler playground were inspected to have less than 6 inches of mulch and rubber tile pieces.

An incident report was not used to document this prohibit item that was left in a child's bag and brought into the center to be unsafe.

Based upon information gathered as a result of this investigation and interviews conducted it was determined a Licensing Incident/Illness Report Form 7239 was not available for review.

Based upon information gathered as a result of this investigation and interviews conducted it was determined that the operation failed to immediately notify the parent on the situation that placed the child at risk.

Based upon information gathered and interviews conducted supervision was determined to have occurred after a three-year child was found in the kitchen unsupervised.

Based upon information gathered as a result of this investigation and interviews conducted it was determined that the operation failed to share an incident report for a parent signature of the incident.

I observed exposed red wires exposed coming out of the wall by the toddler playground gate. A work order was been called in on 8/20/2021 and the building technician corrected at inspection.

In the Infant B Room, the Fire Extingusher guage was in the Red. Last Fire Extinguisher check was 08/02/2021.

Upon reviewing children's records, it was found that 5 out of 10 childrens records was missing special needs statement, 3 out of 10 was missing food allergy specifications, and 1 out of 10 was missing health care providers statement.
