Based on information gathered throughout investigation, a preschool child was put in a highchair and allowed to sleep in it.

Based off DFPS investigation, the operation was unable to provide personnel files to investigator upon request.

Based off information gathered throughout investigation, the director failed to ensure compliance with record keeping requirements. The director failed to produce sign in/out logs, personnel training files, personnel attendance logs, current children's admissions information, and incident reports.

Based off DFPS investigation, the operation was unable to provide employee timesheets to the investigator upon request.

Two out of the four caregivers are short training hours for training year 2022-2023.

Older children were observed walking through the infant room to access their classroom.

The school age classroom did not have a written activity plan to follow.

During the inspection, a caregiver was observed using negative statements and tone when redirecting toddlers and three year olds.

The operation did not have their current healh inspection report available for review.

Nap mats for toddlers, pre-k, and afterschool groups did not have children's names on them or were assigned to children by number.

A caregiver who had unsupervised access to children in care and has been employed over 90 days, does not have current pediatric first aid and CPR training.

Three children did not have current immunization records on file.

A 14 month old was being cared for in a classroom where the oldest child in the room was 35 months. The oldest child was 21 months older than the youngest child. This was corrected when the infant was moved to the infant classroom.

Based on the information obtained during the investigation it was determined that children were out of control. Children were fighting in the classroom.

The operation did not have monthly maintenance checklists of active play space available for review.
