During the course of a monitoring inspection the caregiver responsible for three children ranging in ages from 7 months to 3 years was observed to fall asleep while feeding a child.

Child care home has a pet dog in backyard, which has not had her current Rabies vaccination.

While reviewing primary care giver's personnel file; it was found that she has not completed annual required training; she has completed 2.5 of the 30 hours required.

Two of the four children's records reviewed did not have the physician's signed health statement portion filled in on admission form.

During inspection, primary caregiver admitted to washing bedding once monthly, when asked how often sheets are washed. Minimum standards requires washing of linens on a weekly basis, unless soiled.
