At time of inspection a stationary outdoor play equuipment that contains slides did not have the 6 feet use zone all around the slides.

At time of inspection, four infants were observed sitting in highchairs and were not strapped in.

At time of inspection an air vent in the infant room was observed to have dust.

At time of inspection a provisional caregiver was left alone with children, violating a condition on background. Note: This was corrected at inspection as caregiver was then placed with another caregiver with no conditions.

At time of inspection an infant was placed face down inside their assigned crib. Note: This was corrected during inspection as child was then placed on his back.
