One caregiver did not have proof of training in pediatric CPR.

One child with a diagnosed food allergy did not have a food allergy emergency plan.

Three children with food allergy emergency plans did not have the medication listed on the plan present at the operation.

During inspection, the file drawer with staff records was locked and unable to be opened to verify that annual trainings were being completed.

The last sanitation inspection available for review was from 10/27/21.

The feeding instructions for 2 children had not been reviewed with the parents to ensure all of the required feeding information was obtained. For one child there was no date listed for the last 2 months and there was no parent signature for one month.

The sipping cups/bottles used for infants did not have proper labeling of the childrens first and last initials. There was one sipping cup stored in the refrigerator without a name labeling. *This was corrected at inspection when all cups and bottles were properly labeled as according to the rule.

The operation failed to notify Child Care Regulations changes in the Controlling Persons and that a required person is no longer a Controlling Person for the operation.

The operation failed to notify Child Care Regulations in writing regarding the designated directors departure from service. The operation did not notify Child Care Regulations in writing about the designation of a new director. Licensing learned on 3/8/2021 that the director designated is no longer employed as of January 2021.
