During an investigation, it was found that a child was left alseep on their cot while both caregivers took the other children out of the classroom and down the hall. The child was left unsupervised for around 1 minute and 15 seconds.

During investigation of the videos provided by the adjoining day care and also the video obtained by the main HeadStart office. It was observed that the caregiver kicked the child, raising her (child) legs off the ground and causing her to roll to the side, and lifted another child completely off from the ground by his coat.

During a walk-through of the childcare facility, a group of children younger than 2-years-old were observed having a screen time activity. The screen time activity was immediately turned off when addressed, bringing the facility into compliance. The children were allowed to choose a different activity.
