Observed one child under the age of 2 years old watching Mr. Rachel. This was corrected when the young child was given a different activity to do in a different area of the classroom.

Observed a caregivers file that had been employed since the end of May without orientation training. I discussed orientation being needed within the frist 7 days of employment. The caregiver is no longer employeed at the operation.

On camera, it was observed that a caregiver with conditions on their background check left the classroom with a child to take the child to the office. The caregiver has a condition that states that they cannot be left alone with children. The caregiver no longer works at the operation. It was observed on video that a caregiver grabbed a child by their upper right arm with the child's feet off of the floor and carried the child to the office by the child's arm. This was corrected when the caregiver was removed from the operation and is no longer employeed by the daycare.

There were cleaning supplies observed on the bottom shelves of an accessible and unlocked cabinet in the preschool classroom's bathroom. This was corrected when all of the cleaning supplies were moved out of reach.
The diaper changing mat in the infant room was observed to be torn.
During the monitoring inspection, a 2 month old was observed sleeping in a restrictive device with a blanket on top of them.
An electrical outlet was exposed in the infant classroom. A safety cover was immediately placed over the electrical outlet when addressed, bringing the facility into compliance.

After evaluating safety practices, a child enrolled in care that had been medically diagnosed with a food allergy did not have a food allergy emergency plan signed and dated by the parent. The food allergy emergency plan had been signed and dated by the physician.
Two infants enrolled in care not yet ready for table food did not have written infant feeding instructions updated every 30 days. The most recent update was April 2021.

During a walk-through, a 3-year-old classroom was observed to be out of ratio. According to the specified age group, the maximum number of children to one staff member for this age group is 15. There classroom was over ratio by one child.
