During an investigation it was found that some of the bottles were not labeled. It was found that sometimes the lids of the bottles are labeled.

A caregiver had access to the children in care without a cleared background check. This has been corrected as the person no longer works at the operation. An administrative penalty will be imposed in the amount of 100% of the daily maximum per statute, HRC 42.078(a-2)

Based on the information gathered during the investigation it was determine that the caregiver did not notify anyone at the operation of infant falling off table immediately. This was corrected at the inspection by discussing with the caregiver the immediate threat/risk of an infant falling from equipment
