An emergency plan for a child with a diagnosed food allergy was not observed to be signed and dated by the parent and health care professional.
A 14mo old infant was observed to be cared for in the same room a group of children 2-4 years old.
Upon arrival, an 14 mo old infant was observed sitting in a high chair.According to the caregiver, the child was in the high chair for approximately 10-15 minutes. The child was not fed while in the high chair.
In the refrigerator in the infant room, A bowl of food was observed to be uncovered. NOTE: Corrected during the inspection. The bowl was removed from the refrigerator.
There is a loose screw on the play structure and there is cracked equipment that also contains loose screws on the playground.
A ceiling tile in the hall way contains mold. Paint is chipping from the walls and the base boards are lifting on the walls in two of the classrooms. There is a white wall in the restroom that is unsecure and falling over.
Various objects including a blue bin, cleaning supplies and bags of clothing were placed on a shelve directly over the toilet in the restroom which was unstable. Note: These items were moved during the inspection.
Based on the information obtained during the investigation there is sufficient evidence to support that staff members are pulling the children's ears and lips as a form of discipline.
Based on the information obtained during the investigation, toddlers are strapped into highchairs until parents come to pick them up while other children are allowed to play in the room.
